The following is a list of signs that may indicate that a resident is being abused or neglected. However, not every situation listed below is proof of abuse or neglect, some of the following signs may result from a pre-existing medical condition that the resident is suffering. Be watchful of the following:

  • Resident is not receiving appropriate oral hygiene
  • Resident's hair is unclean
  • Resident is not receiving regular baths
  • Resident is incontinent and is constantly found wet and soiled
  • Resident is immobile and is not turned for periods exceeding 2 hours
  • Resident has redness or bruises on skin, or skin tears
  • Resident's skin is dry and flaky
  • Resident is losing weight
  • Resident has recurrent urinary tract infections
  • Resident has pressure sores (or bedsores)
  • Resident has stiffened joints or atrophy of muscle
  • Resident's medical condition is rapidly declining
  • Resident experiences increased lethargy (sleeping abnormal lengths of time, slurred speech)
  • Resident's room has a foul odor
  • Resident is physically restrained
  • Resident has had excessive falls and facility has not implemented any preventative measures
  • Resident is given very little assistance at meal time
  • Few staff on resident's unit
  • Rarely any staff in resident's room
  • Resident's physical, mental, or psychosocial status has changed and facility has not notified family
  • Resident has an accident resulting in injury and the facility has not notified family
  • Nursing home staff is non-responsive to family's inquiries about resident's medical condition
  • Nursing home staff is non-responsive to family's requests for assistance
  • Family has requested changes in resident's care that nursing facility has not implemented
  • Family or responsible party is not invited to attend resident's care plan meeting
